Targeted Care Packages

Targeted Care Packages support children and young people to transition to a more stable and permanent care arrangement such as Foster care. Each package is customised to the individual needs of the child or young person and can include a wide range of support services, therapeutic supports and other activities that focus on assisting the child or young person to have positive experiences and build meaning, purpose and connectedness within their life.

CareChoice supports children and young people receiving Targeted Care Packages by providing consistent, compassionate and trauma informed staff who support their specific needs. Our focus is to provide a safe, caring, welcoming and homely environment, and actively engage children and young people with appropriate supports so they feel safe and are able to develop healthy positive relationships with others.
