CareChoice Homes is a housing service model that offers ‘forever homes’ to NDIS participants with eligible Specialist Disability Accommodation (SDA) funding. We provide our day-to-day support services in new houses from our collaborating SDA partners.
Our partners are NDIS-registered SDA housing providers offering a wide range of quality, architecturally designed homes across Melbourne, Geelong, Ballarat, Bendigo and other key regions in Victoria.

Modern, architecturally designed houses for people with complex disability support needs are now available in Mount Duneed (near Geelong) and Wyndham Vale. CareChoice is collaborating with one of our NDIS-registered housing partners to offer these houses for eligible SDA participants in sought-after locations close to public transport and amenities. Each home has been designed for two participants.
These safe and secure houses are designed to be ‘forever homes’ for people with a disability, including those who need wheelchair accessible accommodation, who are able to access SDA funding to secure a long-term home. Our aim is to ensure people who are inappropriately housed in a nursing home, community residential unit, or who are living with ageing parents and uncertain about their future, can access new disability housing.
These large three-bedroom houses are designed for two participants with an onsite overnight assistance (OOA) room for a support worker. With wheelchair accessibility, the six-star energy rated houses include features such as extra-wide hallways and doorways, an accessible bathroom, capacity for ceiling hoists and assistive technology, as well as central heating and cooling to accommodate high physical support needs. Our multidisciplinary team of qualified and experienced support staff will provide ongoing, consistent, and round-the-clock support.
In addition to these houses, CareChoice is partnering with NDIS-registered housing providers across Melbourne and key regional areas to offer more forever homes.
These projects will support people across the SDA design categories of high physical support, improved liveability, fully accessible and robust.
Our housing providers are able to build in any suburb/region where a non-gradient block of land can be purchased. We are also working with a housing provider that can purchase and modify an existing house in your preferred suburb or town. If you are seeking an SDA home, then call us on 1300 737 942.
