CareChoice supports clients to re-enter the community from forensic settings

CareChoice is an experienced provider of in home, residential and community support for people with complex needs. For clients re-entering the community from forensic and correctional settings, CareChoice can align its supports to the client’s individual needs as identified by their case manager and complex care team.

CareChoice staff working on forensic re-entry programs receive specific client training so they can work with behaviours of concern effectively and provide targeted support and capacity building for daily activities, during community access, and to support a client’s engagement in therapeutic supports.

CareChoice also works to build strong partnerships with forensic service providers to insure the best therapeutic outcomes for its clients.
